Medical care services include but are not limited to routine office visits for illness, annual physical exams, well-child checks, referrals, immunizations & vaccinations, lab services, sports physicals and more. Dental services include diagnostic and preventive services, fillings, extractions, root canals, extractions, etc. Additional services are medication assistance, chronic disease management, referrals and more. We welcome insured patients, including Medicare and Medicaid, uninsured/underinsured. Sliding fee discount program based on household income is offered.
