Care 4 U Community Health Center is a comprehensive community health center in Liberty City, providing a full range of health care and preventative services to our community’s adult, adolescent, pediatric, and specialty needs populations. Our services include primary medical care, well child visits, immunizations, HIV specialty care, mental health counseling, and substance abuse counseling. Our service delivery model is designed to increase access to care for low-income individuals by including daily outreach and transportation, on-site pharmacy, and access to continuum of care services.
