St. Paul’s PACE San Diego
About this Resource
PACE provides coordinated medical and social services to seniors over 55 who have chronic conditions and require regular medical support, yet wish to continue living in their own home. We offer primary care, therapy services, wellness programs, in-home care, daily activities, and transportation to and from appointments. For seniors with Medi-Cal and no share of cost, all St. Paul's PACE services are free.
St. Paul’s PACE in Downtown San Diego opened in 2008 and was the first PACE program in San Diego County.
