RISE Housing and Support Services is a nonprofit housing agency, which primarily serves adults diagnosed with mental illnesses and addictions. Through a number of residential, supportive, and case management programs, we are licensed to provide trauma informed care and support to individuals across Saratoga County. RISE operates 8 residential programs that offer a continuum of care from community residences to independent supportive housing. RISE partners with Adirondack Health Institute to provide Health Home Care Management services in Saratoga, Warren, and Washington Counties. Last year, our staff provided care coordination to over 1,000 individuals and families to ensure adequate access to needed care and resources.
