Park DuValle Community Health Center, Inc. (PDCHC) is an independent non-profit federally qualified health center (FQHC) in Louisville.
PDCHC offers Adult Medicine, Pediatrics, Women's Health, Dental, and Behavioral/Mental Health services. The second floor of the main office also houses an Insurance/Medicaid Enrollment Office, as well as a WIC office. Case Management services are also available.
PDCHC patients also have access to our in-house lab, x-ray and 340b discount pharmacy. Transportation to and from is available for those who qualify.

PDCHC sees all patients regardless of income and insurance standing, offering a sliding fee scale as well as accepting medicare, medicaid and most major insurances.
