Description

FoundCare is a nonprofit Federally Qualified Health Center that offers pediatric and adult primary care, chronic disease management, behavioral health, dentistry, laboratory services, x-rays, women's health, mammography screening, social services, and on-site pharmacy. Most health insurance plans are accepted. A sliding fee discount program based on household size and income is also available. No one is denied services based on inability to pay. This health center is a Health Center Program grantee under 42 U.S.C. 254b. This health center receives HHS funding and has Federal PHS deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals.
