About this Resource
Foundation for Community Health (FCH) works together with people and organizations to improve the health and well-being of residents in our communities, especially those who have been historically under-resourced.
FCH is a non-profit grantmaking organization dedicated to improving the underlying conditions that contribute to the health of the community at large, often referred to as the social determinants of health.
FCH serves the residents of northwestern Connecticut, eastern Dutchess and southeastern Columbia counties in the New York.
