Corewell Health Grand Rapids Hospitals Healthier Communities is a free prevention and early detection health-screening program. We aim to reduce cardiovascular disease and diabetes, among Kent County residents. In other words, we encourage this at risk population to “improve their health; improve their life.”

This program ensures that participants understand the numbers on their clinical profile and learn how to bring them or maintain them within desired ranges. The program also provides free education on nutritional wellness and physical activity at different sites in the community.

This site is not an urgent care center nor does it offer radiography or laboratory services.
