Community Healthcare Center at Zundy is a school-based Health Center located directly behind Zundy Elementary at the corner of Avenue H and Filmore. Patients of all ages are welcome for medical and behavioral health appointments at this cinic site. We welcome Medicare, Medicaid, most STAR Plans, CHIP and most Insurances. We also have a sliding fee discount based on family size and income, regardless of insurance status.
