CHCY offers integrated primary health care services to all of Yavapai County. Sliding fee discount program available to those qualified, and contracted with most insurance companies including medicare and medicaid. Services include, but not limited to: Medical and Pediatric Care, Dental Care, Reproductive Health, Prenatal Services, Well Woman Healthcheck, Care Coordination, and Behavioral Health.
