4th floor - Admin offices
3rd floor- Adult medicine is on the third floor. Pediatrics includes both behavioral health and regular medical care (internal medicine) is on the second floor. Financial/Eligibility/Insurance services is on the first floor, along with Quest Diagnostics. Dental services are located on the 1st floor. We also have limited behavioral health services at this campus. We have complete adult mental health services at our 152W ST office. Don't hesitate to call our front desk to find out more info we are happy to help you always. We also offer full video health care in addition to a in person visit. Just ask about it when you call us.
