The Black Men’s Health Clinic (BMHC) connect residents in the Eastern Crescent through formal and informal relationships to improve access to healthcare. The Black Men’s Health Clinic provides services via partnerships and collaborations with a referral process for holistic and comprehensive services. Programmatic support includes health education, screenings for high-risk health threats, and outreach for men experiencing homelessness. The clinic identifies patient-needs, and coordinates appointments with warm handoffs or education classes and with partners referring their patients to the Black Men’s Health Clinic for appointment coordination or education classes outside their cultural competency, bandwidth, or area of functionality.
