The Chubbuck clinic offers standard appointments and welcomes same-day appointments through our walk-in clinic service. For patient convenience, we also have an in-house pharmacy. We are honored to be part of the Chubbuck community and to be trusted with your healthcare needs. The Chubbuck clinic is accepting new patients. Our Health West Chubbuck Clinic was established in February of 2014. All patients are encouraged to apply for the Sliding Fee Program which charges fees based on income and family size.
