As your “Patient Centered Medical Home” (PCMH), your Day Kimball family medicine physician and the rest of your primary care team also coordinate and facilitate your care throughout our system. To aid in this effort, we’ve designed our integrated system to allow your primary care team and our walk-in center, emergency department, specialists, diagnostic imaging technicians, laboratory technicians and other healthcare professionals to “talk” about your health through electronic medical records that provide instant access to your medical information.
