Since opening its doors to the South Perry District, Lincoln Heights Area, and surrounding neighborhoods in 2015, CHAS Health’s primary care team has been meeting the needs of individuals and their families. Our Perry clinic also offers a wide range of Women’s Health Services as well as Behavioral Health assistance for adults, children, and adolescents.

In addition, CHAS has Pediatrics, and specialty care such as nutritional services and services for adults 18+ Not only does our on-site pharmacy makes picking up patient prescriptions easy and convenient, our pharmacy staff can answer medication-related questions and help with pain management or diabetes control.
