The Cornell Scott-Hill Health Center is proud to be a Patient-Centered Medical Home (PCMH). A PCMH is a system of care that creates a strong partnership between you and your personalized care team. Your team will be led by your primary care provider and may include nurses, health educators, specialists or any other medical personnel who can help you achieve your health and wellness goals. As partners committed to you and your wellness, we'll work with you and your family to coordinate easy access to care. We'll help you select a primary care provider and will coordinate your care using our electronic health records. We'll involve you in every step of your care plan and teach you how to self-manage chronic conditions in order to stay healthy
