Since 2008, Community Health & Dental Care ensures access to appropriate levels of care for all people in the service area regardless of ability to pay. Our CHDC 800 Heritage Drive location provides dental services and free transportation. Services will vary at our other Pottstown and Barto locations. We accept most major Commercial, Medicare & Medicare supplement plans, Fee for Service Medicaid & Managed Care Medicaid plans, and provide a health care discount for eligible patients. For qualified patients receiving a 100% healthcare discount, the nominal fee for a medical/behavioral health/vision visit is $20, $35 for a dental visit & $5 for a prescription.
