We provide assistance and support for persons with disabilities, living independently in the community. This includes the elderly and others with special health care needs. Our services are provided in the recipient’s home or the community when normal life activities take him or her outside the home.
Our Individualized home supports cover community living services in four categories: Community participation Health, safety and wellness, Household management, and Adaptive skills
At Credent Care, our support services relate to outcomes identified in the person’s support plan when teaching and training are not necessary to achieve these outcomes, and it is the least costly assistance to meet a person's needs.
